Our base monthly rate starts at $9,000 and includes personal supplies. Additional costs may apply based on individual care needs and required services. We can assist with suite furnishing when needed. Private insurance is accepted subject to policy terms and coverage requirements. Residents transitioning to Medicaid must complete a two-year private-pay spenddown period before transitioning, subject to applicable eligibility requirements and approval.

We serve families throughout Olympia, West Olympia, Tumwater, Lacey, and surrounding communities.

To begin onboarding, please email your provider application, referral agreement, credentialing checklist, required documentation, and other onboarding materials. We will review and return completed forms promptly.

We would also appreciate a brief 15-minute introductory call within the next two weeks. Please reply with two or three convenient times. We can discuss your referral workflow, admission criteria, availability, appropriate client profiles, required clinical information, and support for urgent, complex, hospital-to-home, and rehabilitation-to-home transitions.
